OverviewThe Virtual LPN/LVN Home Health nurse administers skilled nursing services to patients requiring intermittent professional services via approved virtual care technology. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician's orders and the established plan of care (POC).This job is fully remote, but you must live in a state that Enhabit does business in.Responsibilities• Perform patient visits in a timely, professional, and appropriate manner per standards of care. Ensure visit time fully meets the needs of the patient.• Coordinate total patient care including interdisciplinary communication with other health care providers, office, and physician.• Contact physician regarding patient changes; receive and write verbal orders.• Render skilled nursing care, as assigned by the registered nurse, according to the physician's orders and in conformance with recognized nursing techniques and procedures, established standards, and branch policies and procedures.• Recognize and interpret symptoms; report patient's condition and assist with or institute remedial measures for adverse developments.• Notify the physician and branch staff of significant changes in the patient's status.• Guide and instruct patient and their families in prescribed activities that are directed toward improving independence and functionality.• Cooperate and assist other health care providers to enhance continuity of care.• Review and update the medication profile; assess patient compliance, understanding of medication regime, contraindications, and side effects.• Ensure that missed patient visits are rescheduled in the same Medicare week; adhere to patient's POC.• Perform all other tasks as assigned.Qualifications• Required to be a graduate of an approved school of clinical education.• Required to be licenseed in the state where they currently practice.• Preferred to havecompact nursing license.• Minimum of 1 years experience as a license nurse is required.• Required to have and maintain CPR certification, as required by position.• Required to have and maintain a valid state driver license in the current state of residence.• Required to have and maintain automobile liability insurance, as required by law.• Previous experience in a home health or hospice setting is preferred.• Previous experience using HomeCare Homebase is preferred.• Oral communication, written communication, fluency in English, active listening.• Demontrated intermediate technology skills. Especially Microsoft office, including word and excel.• Other than for designated full-time remote positions, in-office attendance is essential in order to achieve Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.• Enhabit does not waive this essential function by permitting duties to be performed remotely on occasion to accommodate temporary personal needs, such as appointments, or to operate in crises beyond its control, such as during a natural disaster or pandemic.Additional InformationAt Enhabit, we firmly believe our people are our greatest asset!
