Perfecting Peds is a virtual professional service focused on comprehensive medication management for high-risk and high-cost pediatric patients. They are seeking a licensed pediatric pharmacist to provide remote medication management services, collaborate with healthcare providers, and educate patients and caregivers on medication use.Responsibilities• Conduct thorough medication reviews for pediatric patients, ensuring appropriateness, safety, and efficacy.• Collaborate with healthcare providers to develop and implement comprehensive medication management plans for pediatric patients.• Provide remote consultations to caregivers to address medication-related concerns.• Offer recommendations for medication adjustments, therapeutic alternatives, adherence strategies, etc.• Educate pediatric patients and their caregivers on proper medication use, potential side effects, and the importance of adherence.• Provide guidance on lifestyle modifications and other non-pharmacological interventions to optimize patient outcomes.• Work collaboratively with interdisciplinary healthcare teams to ensure seamless communication and coordination of care.• Participate in case conferences and team meetings to discuss complex cases and develop integrated care plans.• Maintain accurate, timely and up-to-date documentation of medication management interventions.• Ensure compliance with regulatory requirements and organizational policies.• Stay abreast of the latest developments in pediatric pharmacotherapy through ongoing education and training.• Contribute to the development of evidence-based guidelines and protocols for pediatric medication management.• Participate in quality improvement initiatives to enhance the overall effectiveness of medication management services.• Collect and analyze data to evaluate the impact of interventions on patient outcomes.• Other duties as assigned.Skills• Doctor of Pharmacy (PharmD) from an accredited program; active pharmacy license required.
