Job Summary and ResponsibilitiesThis position will be within Deaconess Gateway Hospital.This position does require Home Health experience or prior navigator experience in a post-acute setting such as ALF/SNF/ILFAs the Health at Home Navigator (HHN), your expertise in home-based services is essential to ensuring continuity of care for patients transitioning from acute care to home.
