This position functions remotely, but requires strong clinical judgment, communication skills, and a collaborative mindset to ensure continuity of care.Your Day-to-Day Work• Provide nurse telephone triage using clinical judgment, standardized protocols, and decision-making tools.• Assess patient symptoms and determine urgency of intervention, escalating to providers as needed.• Educate patients and families about diagnoses, treatments, and prescribed medications.• Review and triage abnormal cardiac test results, follow through on patient problems like Afib, blood pressure, and post-operative concerns.• Document interactions accurately in the EMR and maintain compliance with HIPAA and CSC standards.• Serve as a clinical liaison between providers, community partners, and clinic staff.• Function as a resource for patients, families, and support staff in cardiac health.• Collaborate in policy and workflow development to improve care delivery.Environment and Shift DetailsThis is a Per-Diem remote position, with initial onsite training required in Olympia WA . Occasional travel and intermittent onsite attendance may be required.
