This position is ideal for individuals who possess a PharmD degree and have proven experience with HEDIS medication adherence initiatives. In this role, you will play a critical part in enhancing medication adherence, promoting positive patient outcomes, and ensuring adherence to Medicare and HEDIS quality standards.Key Responsibilities• Spearhead initiatives aimed at improving medication adherence and related pharmacy quality measures, with a focus on antihypertensive, cholesterol-lowering, and diabetes medications.• Analyze medication adherence metrics to identify care gaps and implement targeted strategies to elevate adherence rates.• Collaborate effectively with healthcare providers, care managers, and other stakeholders to formulate and execute plans that foster adherence to prescribed therapies.• Conduct Medication Therapy Management (MTM) sessions to identify and address barriers to adherence, providing essential patient education for comprehension of medication regimens.• Engage directly with patients to resolve medication adherence issues, including financial concerns, potential side effects, and complex dosing schedules.• Employ motivational interviewing and other engagement techniques to assist patients in maintaining adherence to their medication plans.• Monitor and track HEDIS performance metrics, generating insightful reports to assess the effectiveness of adherence initiatives.• Utilize electronic health records (EHR) and pharmacy data to pinpoint at-risk patients and devise actionable plans for improving adherence.• Regularly communicate progress toward HEDIS objectives to leadership, providing actionable recommendations for continued enhancements.• Act as a subject matter expert on medication adherence within the organization, offering training and support to fellow pharmacists and clinical staff on best practices.Required Skills• Doctor of Pharmacy (PharmD) degree from an accredited institution.• Active pharmacist license in Georgia.• Minimum of 2 years of experience in HEDIS medication adherence, quality improvement, or population health initiatives.• Strong familiarity with HEDIS measures, particularly those pertaining to medication adherence.• Proficiency in electronic health records (EHR) and data analysis tools.• Exceptional communication, problem-solving, and organizational skills.• Ability to work autonomously as well as collaboratively within a team-oriented environment.• Experience in motivational interviewing or similar patient engagement techniques is a plus.Qualifications• Background in a managed care setting, pharmacy benefits management (PBM), or a healthcare quality improvement role is preferred.• Certification in Medication Therapy Management (MTM), Certified Geriatric Pharmacist (CGP), or other relevant credentials is advantageous.Career Growth OpportunitiesAt our organization, we prioritize professional development and offer a variety of resources to support your growth.
