If you are a licensed LPN/LVN with a passion for healthcare innovation, patient outcomes, and a knack for technology, we invite you to apply.About the Role:• Serve as the primary point of contact for patients on our RPM365 platform, providing exceptional support and guidance.• Assist patients with device setup and app connectivity, ensuring seamless integration with our system.• Engage with patients to promote care plan adherence, schedule monthly check-ins, and monitor patient data to identify trends and potential risks.• Educate patients through shared resources, empowering them to make informed decisions about their health.• Collaborate with healthcare providers to notify them of critical alerts and ensure timely interventions.• Participate in virtual team meetings to stay updated on best practices, new technologies, and company initiatives.Requirements:• Active nursing license in California (multistate/compact license preferred).• Knowledge of chronic conditions and related monitoring devices.• Proficiency with technology, computer platforms, and smartphones.• Excellent verbal and written communication skills.• Comfortable with high-volume patient outreach and engagement (125+ conversations per week).• Highly organized, detail-oriented, and team-focused.Preferred Qualifications:• Experience in telehealth, patient monitoring, education, or customer service.• Additional language skills are a plus.What We Offer:• Competitive salary: $50,000 - $54,000 (DOE) + performance bonuses.• Flexible schedule: Monday - Friday, 9:00 AM - 5:00 PM (Remote).• Comprehensive benefits: PTO, sick leave, holidays, health insurance, and 401(k).• Paid remote training provided to ensure your success.• Opportunities for advancement within the company.RPM Healthcare is an equal opportunity employer, committed to creating an inclusive environment for all employees.
