This role involves monitoring up to 12 patients at a time via large video screens, identifying abnormal behaviors or safety risks, and escalating concerns to the care team promptly. While primarily virtual, this position may require direct patient interaction when needed. Working under the direction of a Registered Nurse, the associate supports safe, compassionate, and patient-centered care.Essential Functions• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.• Recognizes the patients, their family/designated care partner(s) in providing safe, compassionate and coordinated care based on respect for patient's personal preferences, priorities, needs, and cultural beliefs and values.• Communicates with and supports the interprofessional team, patient, family and designated care partners.• Provides observation for safety of assigned patient.• Reports observations of changes in patient condition or behavior to appropriate staff.• Completes required reports and documentation• This position requires providing service to the following age population(s) pediatric through frail elderly in a manner that demonstrates an understanding of the functional/developmental age of the individual served.Competencies (Knowledge, Skills, and Abilities Required):Interpersonal Relationships -Knowledge of the techniques and the ability to work with a variety of individuals and groups in a constructive and collaborative manner.Patient Rights -Knowledge of principles and practices associated with the rights and responsibilities of patients and caregivers.Health Information Documentation -Knowledge of the relevance of health information and appropriate documentation methods, ability to record medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, procedures, tests, and medical results in written or electronic form.Patient Safety -Knowledge of the risks faced by patients in a healthcare environment, and the ability to take steps to reduce these risks.Life Support -Knowledge of the process taken to restore and maintain essential physiological functions, and the ability to apply these processes in a patient care situation.Accuracy and Attention to Detail -Understanding of the necessity and value of accuracy and attention to detail, ability to process information with high levels of accuracy.Minimum Education and Experience Requirements:Education:High School Diploma or equivalent preferredANDExperience:Previous related experience preferredCNA or PCT licensure preferredMinimum Certifications, Registration or License Requirements:CNA or PCT licensure preferredAdditional InformationExcellent interpersonal and customer service skills and the ability to de-escalate patients who are agitated in a way that preserves patient dignity required.
