About the positionResponsibilities• Contact patients identified with incidental and/or unexpected findings to offer follow-up options.• Provide personalized nurse navigation interventions to patients requesting additional information or referrals.• Assess patient educational levels and psychosocial factors that may impede care.• Ensure efficient use of provider time by recommending appropriate follow-up intervals and avoiding duplicate requests.• Assist primary care providers (PCPs) in ensuring patients receive appropriate follow-up care.• Obtain additional imaging only at the specific request of the PCP.• Educate patients on the follow-up process and direct them to the correct provider.• Utilize software programs to track assignments and maintain communication with colleagues.• Maintain knowledge of applicable laws, regulations, and compliance programs.Requirements• Graduate of an accredited school of Nursing with a Bachelor of Science Degree in Nursing.• Three to five years of progressive clinical nursing experience.• Current RN license in the State of Michigan.• Problem-solving skills and ability to initiate creative solutions.• Demonstrated ability to navigate various software applications effectively.• Effective communication skills with medical staff and other health providers.• Working knowledge of Radiology procedures and protocols is preferred.• Effective organizational skills.Nice-to-haves• Knowledge of Windows-based programs.• Experience in a high-stress environment.Benefits• Part-time flexible schedule• Opportunity to work remotely• Supportive work environment focused on diversity and inclusion
