Your role bridges communication between patients, families, providers, and hospital departments, ensuring timely, efficient, and compassionate care across the continuum. You will also serve as a resource for staff education and act as a liaison for the surgical team.Key Responsibilities Include:• Guiding patients and families through complex care plans with clarity and compassion• Coordinating interdisciplinary communication and services to ensure continuity of care• Identifying educational needs and designing training content for clinical teams• Acting as a clinician, advocate, educator, and consultant to optimize outcomes and patient satisfactionQualifications:Bachelors Degree in Nursing Required.
