This is a field role, and you must reside in Monroe or Phillips counties. You will interact with members via phone or home visits, engage them in care planning, and coordinate services with healthcare providers and partners. Your strong communication and interpersonal skills will enable you to build trust with our members and their families, while your attention to detail and organizational skills will ensure that their care plans are accurately documented and implemented.Key Responsibilities* Provides outreach to members via phone or home visits to engage them in care planning and discuss next steps, resources, and ongoing education* Coordinates care activities based on the care plan and works with healthcare providers and partners to accommodate changes or progress* Serves as a support on various member and/or provider inquiries, requests, or concerns related to care planning* Communicates with care managers, practitioners, and others to facilitate member services and ensure continuity of care* May support performing service assessments/screenings for members and documenting their care needs* Supports documenting and maintaining member records in accordance with state and regulatory requirements* Follows standards of practice and policies compliant with contractual requirements and regulatory guidelines and standards* Ability to identify needs and make referrals to Care Manager, community-based organizations, and Disease Manager* Provides education on benefits and resources available* Performs other duties as assigned* Complies with all policies and standardsEssential Qualifications* High School diploma or GED* 1-2 years of related experience in healthcare or social services* Strong communication and interpersonal skills* Ability to work independently and as part of a team* Attention to detail and organizational skills* Ability to identify needs and make referrals to Care Manager, community-based organizations, and Disease Manager* Basic computer skills and proficiency in electronic health records (EHRs)Preferred Qualifications* Bachelor's degree in a related field (e.g., social work, nursing, healthcare administration)* Experience working with vulnerable populations (e.g., elderly, disabled, low-income)* Knowledge of healthcare policies and regulations* Certification in a related field (e.g., Certified Care Manager, Certified Social Worker)Skillsand Competencies* Strong communication and interpersonal skills* Ability to work independently and as part of a team* Attention to detail and organizational skills* Ability to identify needs and make referrals to Care Manager, community-based organizations, and Disease Manager* Basic computer skills and proficiency in electronic health records (EHRs)* Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities* Strong problem-solving and critical thinking skills* Ability to maintain confidentiality and handle sensitive informationCareer Growth Opportunities and Learning BenefitsAt Centene, we are committed to the growth and development of our employees.
