As a Case Manager RN, you will play a critical role in assessing, planning, implementing, and coordinating case management activities with members to evaluate their medical needs and facilitate their overall wellness. This is a full-time telework role, with occasional travel required to meet members in the Detroit area and participate in office-based meetings.Key Responsibilities• Conduct comprehensive assessments of referred members' needs/eligibility and determine approach to case resolution and/or meeting needs by evaluating member's benefit plan and available internal and external programs/services.• Apply clinical judgment to the incorporation of strategies designed to reduce risk factors and address complex clinical indicators which impact care planning and resolution of member issues.• Perform crisis intervention with members experiencing a behavioral health or medical crisis and refer them to the appropriate clinical providers for thorough assessment and treatment, as clinically indicated.• Provide coaching, information, and support to empower members to make ongoing independent medical and/or healthy lifestyle choices.• Help members actively and knowledgeably participate with their provider in healthcare decision-making.• Monitor, evaluate, and document care in collaboration with the member and their care team to meet the member's goals.• Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies.Essential Qualifications• Current/Active unrestricted RN licensure in the state of MI.• Willing and able to travel up to 15% of the time within Detroit and surrounding areas to meet members face-to-face. Reliable transportation required; mileage is reimbursed per our company expense reimbursement policy.• Must reside in one of the following counties: Berrien, Cass, Van Buren, Kalamazoo, St. Joseph, Branch, Clinton, Eaton, Ingham, Jackson, Hillsdale, Lenawee, Livingston, Washtenaw, Monroe, Oakland, Macomb, or Wayne.• 4+ years of clinical experience post-bachelors as an RN in a hospital setting or alternative care setting such as ambulatory care or outpatient clinic/facility.• 1+ year of case management experience.Preferred Qualifications• Experience using Microsoft Office products including Word, Excel, Teams, and Outlook.• Experience with complex disease management ranging from children to adults.EducationBachelor's in Nursing required.Compensation and BenefitsThe typical pay range for this role is $60,522.80 - $129,615.20. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors.
