Specific tasks, responsibilities or competencies may be documented in the incumbent’s performance objectives as outlined by the incumbent’s immediate supervisor or manager.• Provide expert guidance on pain and symptom management tailored to individual patient needs and actively contribute to enhancing patient comfort and dignity during end-of-life care.• Collaborate directly with nurses and hospice staff to develop effective, cost-conscious medication care plans.• Educate healthcare professionals and caregivers on medication usage and adverse event prevention.• Accurately document clinical interventions and patient outcomes in electronic health systems. Ensure all encounter documentation is complete, clear, and ready for transcription/processing.• Maintain and update patient medication profiles and records in accordance with HIPAA and regulatory requirements.• Consistently meet or exceed expected productivity benchmarks.• Demonstrate exceptional communication and interpersonal skills in a remote or hybrid work environment.• Maintain a professional, empathetic tone in all patient and team interactions.• Engage with a multidisciplinary care team in a fast-paced call center environment.• Support team success through shared learning, collaboration, and knowledge sharing.• Participate in weekend rotations and contribute to team coverage as needed.Marginal or Additional Functions• Protects company’s intellectual property and the privacy of customers’ and patients’ data• Performs other related duties as assigned or apparent.Supervisory and Managerial Responsibility• This position does not have supervisory or managerial responsibilities.Knowledge, Skills & AbilitiesEducation, Licensure or Certification:• PharmD or B.S. in Pharmacy from an ACPE-accredited school of pharmacy• Active pharmacy license in home state (Pennsylvania or Tennessee)• Pharmacy licenses in other states or willingness to obtain additional licensesWork Experience or Related Experience:• Experience in hospice, palliative care, or oncology settings preferred• Previous experience working in a call center or remote clinical consultation environment preferred• Familiarity with pharmacy benefit management (PBM) services preferredSpecialized Knowledge, Skills & Abilities:• Demonstrates competency in equianalgesic dosing, generic and therapeutic substitution, formulary management and prior authorization procedures• Demonstrates understanding, competency and adherence to all state and federal pharmacy practice rules and regulations• Strong clinical assessment and decision-making skills• Excellent written and verbal communication skills• Proficient typing and computer skills are paramount to employee satisfaction and success while fulfilling responsibilities• Excellent listening skills• Proven ability to manage multiple projects simultaneously while paying strict attention to detail• Ability to maintain confidentiality, and to exercise discretion and good judgment• Excellent interpersonal and customer service skills• Excellent organizational skills and attention to detail• Commitment to delivering high-quality, compassionate patient care• Proficient in Microsoft Office tools and electronic health record (EHR) systemsEquipment:• Working knowledge of a PC, business and communications software (MS Office) and web-based tools is requiredTravel Requirements and Conditions• Limited travel may be requiredWork Environment, Conditions, and Demands• Work is performed in a remote or hybrid office environment.• Remote work Internet Policy Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:• At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested• Satellite, cellular and microwave connection can be used only if approved by leadership• Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA informationPhysical Requirements and DemandsNote: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions as described.• This position entails a high level of work performed at a computer terminal and telephone or headset equipment throughout the day• Regular eye-hand coordination and manual dexterity is required to operate office equipment• Ability to lift up to 30 lbs.Additional Position Information• No additional information is applicable
