As the Pharmacist, Medication Management at Aledade, you'll drive meaningful patient outcomes while transforming healthcare delivery. You'll be at the forefront of value-based care, partnering with field teams, interdisciplinary care members, and practices to optimize medication strategies that directly improve patient lives. You'll guide critical transitions of care and elevate quality measures, serving as the clinical expert who bridges the gap between complex pharmaceutical needs and real-world care delivery. This role offers the opportunity to shape how Aledade's network approaches medication management, ensuring every patient receives the right care at the right time.This Pharmacist will cover our Delaware markets, but candidates should be comfortable working remotely/work from home anywhere within the US. However, current and valid state pharmacist licensure in the state of Delaware is required.Primary Duties:• Quality measure performance - this work involves leveraging current data to identify patients not meeting quality measures and working in collaboration with the practice to overcome patient barriers where possible. This work may also include identifying and implementing pharmacy cost-saving opportunities for patients or practices.• Transitions of care - Perform comprehensive medication reviews as a part of transitions of care, address polypharmacy, adverse drug reactions (ADRs), and deprescribing opportunities. Prevent and resolve medication-related problems and make appropriate evidence-based, patient-centered medication and therapy recommendations back to practice care teams.• Serve as a subject matter expert in areas of business needs (e.g. helping with workflows and communication).• Development and delivery of learning materials for clinicians, practices and patients to close knowledge gaps related to quality measures.• Other duties as assignedMinimum Qualifications:• Doctor of Pharmacy (PharmD) degree or Bachelor’s degree in Pharmacy.• Must be licensed in the state of practice.• Graduate of an accredited School of Pharmacy.• Strong communication skills with healthcare professionals and patients.• In depth knowledge of medication adherence, medication spend and pharmacy quality measures.• Experience with analyzing pharmacy data and identification of gaps in care.• Ability to organize and articulate pharmacy performance to market and senior leaders.• Completion of a pharmacy residency or equivalent (3 or more years of experience in value-based care or community pharmacy practice)Preferred Qualifications:• Value-based care and quality measure expertise, analyzing data and creating strategies based on key takeaways.Licenses & Certifications:• Current and valid state pharmacist licensure in the state of Delaware.Physical Requirements:• Prolonged periods of sitting at a desk and working on a computer.SalaryRange: $98,000 to $130,000 base + bonus + equityCompensationfor the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience.Who We Are: Aledade, a public benefit corporation, exists to empower the most transformational part of our health care landscape - independent primary care.
