It is not just what we do - it is who we are.Overview of ResponsibilitiesProvides Telehealth Services: Engages with patients and caregivers virtually via phone or video calls, offering guidance, monitoring, and support for their health concerns and recovery plans.Remote Patient Monitoring: Utilizes digital health tools to monitor patient vital signs, symptoms, and progress remotely, ensuring prompt intervention when necessary.Virtual Care Coordination: Collaborates with multidisciplinary healthcare teams through virtual platforms to plan and implement patient care, maintaining communication with physicians, therapists, and other providers.Patient Education: Uses virtual platforms to educate patients and families about disease management, treatment plans, and self-care techniques, ensuring comprehension and offering continuous support.Virtual Rounding: Participates in virtual rounding with the healthcare team to assess patient status, adjust care plans, and provide updates on progress.Documentation: Records virtual care interactions in the patient's electronic medical record (EMR), ensuring it meets the hospital's documentation standards and regulatory requirements. Including but not limited to care plan documentation, medication double-check, admission history, and medication reconciliation.Technology Proficiency: Demonstrates competence in the use of virtual care technologies and other digital health tools to provide safe and effective patient care.Virtual Support: Provides emotional and psychological support through virtual check-ins, helping patients manage the stress and anxiety that may accompany their conditionCompliance with Virtual Care Guidelines: Adheres to all regulatory requirements, confidentiality policies, and hospital standards related to virtual care delivery.Directs and delegates care based on identified need, skill and competence of other care providers.
