Since 2015, we have been delivering a range of evidence-based, person centred interventions and services, including: NHS Health Checks, Smoking Cessation, Weight Management and Type 2 Diabetes Prevention and Treatment.</p><p></p><p><u>Integrated Health</u></p><p>Our One You Kensington, Chelsea, and Westminster service helps its local residents live healthier lives and reduce their chances of developing serious illnesses. Our support includes guidance on moving more, being tobacco free, eating well, drinking less, and losing weight.</p><p></p><p><b><u>What is the role about?</u></b></p><p>Health & Wellbeing Coaches are critical to the successful delivery of Reed Wellbeing’s world class, evidence-based health and wellbeing behavioural lifestyle interventions. In this role, you’ll support a caseload of participants by helping them identify and set realistic health goals and develop the knowledge, motivation, and skills needed to make sustainable lifestyle changes and better manage their conditions.</p><p></p><p><b><u>Your day-to-day responsibilities will include:</u></b></p><p></p><p><b><i>Taking Ownership</i></b></p><ul><li>Delivering engaging 1:2:1 or group-based health and wellbeing interventions to a caseload of participants either in a face-to-face, remote, or digital setting</li><li>Support participants in developing tailored SMART support plans with the aim to achieve sustainable behavioural and lifestyle improvements</li></ul><p></p><p><b><i>Working Together</i></b></p><ul><li>Liaising with healthcare professionals including GPs to promote and generate referrals to the service</li></ul><p></p><p><b><i>Being Fair, Open and Honest </i></b></p><ul><li>Capturing key metrics at different points in the participants journey and progress (e.g. weight, alcohol intake) on Reed Wellbeing systems</li></ul><p></p><p><b><u>What’s in it for you? </u></b></p><p>A generous base salary (pro-rata for part time), and great flexibility with your working schedule, we offer training, development and progression opportunities.
