PRN Nurse will travel to assigned client locations to perform Initial Admissions and follow up visits via Assessments within Client place of residence. RN will evaluate patient’s condition, completing appropriate clinical documentation under the direction of the Nursing Supervisor or Director. RN will implement and update Patient Care Plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems, document, and report to Nursing Supervisor.
